NATIONAL MUAY THAI ASSOCIATION - DBA: NMTA WAVIER In consideration of being allowed to participate in any way AT NATIONAL MUAY THAI ASSOCIATION - DBA: NMTA programs or activities, including but not limited to: Martial Arts, Boxing, Kickboxing, Muay Thai, Jiu Jitsu, MMA, Karate, Wrestling, Dance and Tricks, Yoga, Pilates, Bootcamp, personal training, general fitness training, weight lifting, Hitting the bag and including all related events and activities, and including the use of all equipment and facilities both supervised and unsupervised, the undersigned acknowledges, appreciates, and agrees that: The risk of injury from the activities involved is significant, including the potential for permanent paralysis and death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist; 1) I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation. 2) I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest instructor or official immediately. 3) I represent that I am in good health and have no disability, impairment, injury, disease, or ailment, preventing me from engaging in exercise or which could cause increased risk of injury or adverse health consequences to myself or others as a result of exercise. I agree that I will not participate in any activities while under the influence of alcohol and/or illegal drugs, and/or legal/prescription drugs which may affect my ability to participate safely. I assume full responsibility for the use of the facilities and my participation in all activities. Physical examinations by participant's physician are recommended before commencing any exercise program. I represent that there is no medical or physical condition that would preclude the use of Maximus Muay Thai and Fitness facilities and programs, and I represent that I have not been instructed by a physician not to use Maximus Muay Thai and Fitness facilities or any similar facility. 4) I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS NATIONAL MUAY THAI ASSOCIATION - DBA: NMTA, their officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, volunteers, contractors, advertisers, and if applicable, owners and lessors of the premises (“RELEASEES”), WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, to the fullest extent permitted by law. I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT. 5) For the health and safety of myself and my training partners, I willingly agree to comply with following hygiene and grooming requirements for participation in any activity at NATIONAL MUAY THAI ASSOCIATION - DBA: NMTA: Maintain personal cleanliness (bathe daily, brush teeth, use deodorant, etc.). Wash hands after eating, or using the restrooms. Maintain clean and trimmed fingernails. Secure long hair (hair must be tied back to prevent potential injury). Clothing and gear must be clean. Workout clothing (including gis and uniforms) should be washed after each use. Wash and air-out equipment after each use. Do not share equipment. Street shoes are not allowed in the gym. Clean/dry indoor shoes are required for lifting weights, using cardio equipment, and for classes requiring shoes. Clean/dry flip flops are suggested for use in locker/bath rooms. No jewelry should be worn during training. Open sores or cuts or blisters must be covered at all times during training. Do not train if you are sick or have a skin infection (or suspect that you may have a skin infection or rash). Always shower as soon as possible after training. Rules / Gear Requirements: Basic Amateur Muay Thai Rules apply Shins and Headgear - Any is fine, the best you feel comfortable sparring in. No Elbows allowed. Gloves - Adult 16oz / Child ANY Wraps - Regular gym hand wraps are fine, no gauze and/or tape is required. Sweep / Dump - Sweeps and dumps are allowed, but only one strike one step. Please Bring: Cup, Mouth Guard, Shins, Head Gear and Gloves - Chest Protection will be provided.
